Job Opening: Full-Time Patient Navigator/Community Outreach Worker

Caring Connections, a cooperative women’s health program of the Bangor YMCA and Eastern Maine Medical Center, seeks to hire a full-time Patient Navigator/Community Outreach Worker.

Responsibilities:

• Providing education on early detection and cancer risk reduction in the community with a focus on rural populations and school settings,

• Identifying and enrolling eligible women in the no-cost breast and cervical health program and assisting them to obtain all needed screening and diagnostic services.

• Providing support to individuals diagnosed with cancer including accompaniment to appointments

• Assisting with Caring Connections cancer support programs, groups and retreats

Requirements:

• Bachelors Degree in health, social services or related field

• Travel throughout Penobscot and Piscataquis Counties, to include providing transportation to program recipients when needed.

• Able to lift 30 lbs. and walk up two flights of stairs.

• Occasional nights and weekends required.
